Local Pay: $55.00 – $61.00 per hour
Weekly: $2,062.50 – $2,287.50
Travel Pay: $60.00 – $73.00 per hour
Weekly: $2,250.00 – $2,737.50

"Navitas Healthcare, LLC" is seeking a dedicated and clinically skilled Licensed Master Social Worker (LMSW) to support inpatient medical services in Manhattan, NY . This contract role focuses on providing psychosocial support and discharge planning across Inpatient Medicine, Surgery, and Rehabilitation units. The LMSW will collaborate closely with interdisciplinary clinical teams to ensure comprehensive patient-centered care and smooth transitions across levels of care.
Psychosocial Assessments: Evaluate patients in medicine, surgery, and rehabilitation units to identify emotional, social, and financial needs impacting care and recovery.
Discharge Planning: Coordinate complex discharge plans, including home care services, skilled nursing facility placement, and community resource referrals.
Crisis Intervention: Provide counseling and emotional support to patients and families during acute illness, hospitalization, or end-of-life situations.
Interdisciplinary Collaboration: Participate in daily multidisciplinary rounds with physicians, nursing staff, and allied health professionals.
Clinical Documentation: Maintain accurate, timely documentation in compliance with hospital policies and NYSED standards.
Master’s Degree in Social Work (MSW) from an accredited institution
Active Licensed Master Social Worker (LMSW) credential issued by the New York State Education Department (NYSED)
Minimum of one (1) year of professional LMSW experience
Hospital-based experience in inpatient medicine, surgery, or rehabilitation required
